In Majid Majidi’s drama Beyond the Clouds (2017), Khatter landed his first major role. For his portrayal of a heroin dealer, he was awarded the Filmfare Award for Best Male Debut. Khatter played the drug dealer Amir in Majid Majidi’s 2017 film Beyond the Clouds, which was his first adult acting role. Next, Khatter portrayed the lead in the love drama Dhadak (2018), directed by Shashank Khaitan and reimagined as a Marathi film Sairat (2016), with Janhvi Kapoor, a novice, costarring. It narrates the tale of two young lovers in rural Rajasthan who, as a result of caste prejudice, encounter political resistance to their union. While finding “a soulfulness to [Khatter] that’s at odds with Hindi cinema’s current penchant for bratty, hyperactive onscreen male personas,” Uday Bhatia of Mint criticised Dhadak for lacking the depth of the original film. The Indian Express’ Shubhra Gupta didn’t like the movie either, but she did call Khatter a “natural” performer and praised his “mobile, expressive face”. Dhadak became a commercial success, earning over ₹1.1 billion (US$14 million) globally.

Khatter played alongside Tabu in Mira Nair’s British miniseries A Suitable Boy (2020), which was based on Vikram Seth’s novel of the same name The BBC One broadcast it in six segments. Ishaan Khatter was interested to the idea because it portrayed a romance between a young guy and an elderly woman, which he believed would combat the stigma associated with such relationships in society.
